Impact of medication bedside delivery program on hospital readmission rates

BackgroundSince the establishment of Hospital Readmission Reduction Program by Centers for Medicare and Medicaid Services, reducing readmission rates has been a priority health care institutions. Many institutions have developed services to combat high rates, including bedside medication delivery programs, which demonstrated reductions in 30-day patients who used these services.ObjectiveTo evaluate impact system–based programs on at low moderate risk hospital readmission.MethodsA single-center retrospective cohort study conducted adult low-to moderate–transitions (TOC) status with unplanned admissions large academic medical center between January 1, 2017, 2019 service or an outside pharmacy. The TOC was defined using historic institutional definitions. Patients least 2-day stay were discharged home from select primary included. outcome 2 groups.
